甲状腺癌是一种起源于甲状腺滤泡上皮细胞或者滤泡旁C细胞的一种常见的内分泌恶性肿瘤,其发病率在世界许多地区一直呈上升趋势,我国城市女性甲状腺癌的发病率位居女性所有恶性肿瘤的第4位,并将以每年20%的速度持续增长[1]。依据细胞来源不同,甲状腺癌可分为髓样甲状腺癌(medullary thyroid cancer,MTC,3%~5%)和非髓样甲状腺癌(non-medullary thyroid cancer,NMTC)。MTC来源于滤泡旁C细胞,遗传性MTC占20%~25%,可分为MEN2A、MEN2B和家族性髓样甲状腺癌(familial medullary thyroid cancer,FMTC)[2-3]。滤泡细胞来源的甲状腺癌按照组织学形态分4类:乳头状甲状腺癌(papillary thyroid cancer,PTC),占80%;滤泡状甲状腺癌(follicular thyroid cancer,FTC),占10%~15%;低分化甲状腺癌(poorly differentiated thyroid cancer,PDTC),<2%;未分化甲状腺癌(anaplastic thyroid cancer,ATC),<2%。PTC和FTC是分化良好的甲状腺癌(well-difffferentiated thyroid cancers,WDTC),占甲状腺癌的绝大多数,病程缓慢,通常可治愈,预后一般较好[4-5],但某些亚组患者中的复发率也达20% ~30%。未分化甲状腺癌占比少,但病死率高;PDTC的病程介于分化型甲状腺癌(differentiated thyroid cancer,DTC)和ATC之间[6-7]。准确评估甲状腺癌的分型以及复发风险决定了对患者的最适当治疗管理方案。核心的挑战是如何平衡与治疗相关的利益和与治疗相关的危害。


多种基因的变异是甲状腺癌变的一个重要指标[8],甲状腺癌的起始和进展涉及多种遗传和表观遗传改变,其中导致MAPK和PI3K-AKT信号通路激活的突变至关重要,体细胞突变和其他分子改变已被认为是有助于甲状腺癌的诊断和判断预后的标志物,并已被引入临床实践,为管理甲状腺结节患者提供了一种有价值的工具[9]。基因检测已被写入《甲状腺癌诊疗规范和指南》中。《甲状腺癌诊疗规范2018版》[1]和《美国国立综合癌症网络(NCCN)指南》[10]中均推荐对于意义不明的非典型细胞或意义不明的滤泡性病变、滤泡性肿瘤或可疑滤泡性肿瘤进行分子检测。当前,甲状腺癌的基因检测已经逐步进入到临床应用阶段,但尚存在挑战,如技术重复性、对检测平台要求较高、检测费用较高等,未来基因检测需要朝着更加准确、快速、费用更低的方向发展。


一、甲状腺癌基因检测常见的分子标志物


大多数甲状腺结节是良性的,临床的挑战是准确和快速鉴别结节癌。目前,采用细针穿刺(fine-needle aspiration,FNA)活检对甲状腺结节进行取样并对收集的细胞进行细胞学检查是最准确和最广泛使用的诊断方法,FNA在大多数情况下能够明确区分恶性和良性结节,但仍有约25%的结节不能通过FNA细胞学诊断[11-15]。国内将2017版Bethesda系统(TBSRTC)[16]作为甲状腺细胞病理诊断标准。根据诊断标准,细胞学分别诊断为:标本无法诊断、良性病变、意义不明确的细胞不典型病变或滤泡性病变(AUS/FLUS)、滤泡性肿瘤或可疑滤泡性肿瘤(FN/SFN)、可疑恶性肿瘤(suspicious for malignant cell,SMC)和恶性肿瘤(MC)6种类型。一项对美国2002-2010年发表的11项大型研究的综述结果显示,62%~85%的FNA为良性,1%~8%为恶性,10%~26%为不确定性,1%~11%不能诊断。在接受手术的FNA细胞学检查不确定的患者中,约14%~48%为恶性肿瘤,这种发生率较高[17],其中Bethesda Ⅲ类AUS/FLUS恶性肿瘤占5%~10%,Bethesda Ⅳ类FN/SFN中20%~30%为恶性,Bethesda Ⅴ类可疑恶性肿瘤SMC中50%~75%为恶性肿瘤[18]。“甲状腺癌诊疗规范2018版”中推荐Bethesda Ⅲ~Ⅳ类FNA无法确诊的甲状腺结节进行分子检测。此外,Bethesda Ⅲ~Ⅴ类不确定的诊断也给临床管理带来了巨大挑战,因为良恶性的诊断决定了甲状腺结节管理方案的制定[1]。此外,既往对FNA细胞学检查不确定的甲状腺结节的治疗常采用诊断性肺叶切除术或甲状腺切除术,该方法有一个明显的缺点,即许多患者要承受过度治疗的手术风险、手术并发症、成本和工作时间,最终证明是良性甲状腺结节。


在过去的20年中,甲状腺癌的分子检测临床转化研究取得了重大进展,寻找合适的分子标志物来指导手术或观察FNA不确定甲状腺结节的患者,是近年来甲状腺结节研究的圣杯。研究结果表明MAPK和PI3K-AKT信号通路的异常增强驱动甲状腺癌的发生和发展,常见的分子检测标志物有BRAF和RAS基因的点突变以及RET/PTC和PAX8/PPARγ染色体重排等[19-22]。在将常见的分子检测标志物总结如下(表1)。

1. BRAF基因突变:BRAF是一种丝氨酸-苏氨酸激酶,经RAS结合激活后转运到细胞膜上,导致MAPK激酶等MAPK信号通路下游靶点的磷酸化和活化。在甲状腺癌中BRAF被突变类型有点突变、小缺失或插入和染色体重排,其中点突变BRAF V600 E(c.1799 T〉A;P.600 Val〉Glu)突变最为常见,占甲状腺癌中发现的所有BRAF突变的98%~99%[20, 23-24]。该突变在40%~45%的PTC中被发现[25]。有学者发现在显示存在分化良好的乳头状癌区域的20%~40%的PDTC和30%~40%的ATC中也存在该突变[26-29],提示这种突变发生在肿瘤去分化的早期,同时由于该突变在FTC中比较罕见[25, 30],因此对甲状腺结节FNA来讲,BRAF突变是一个高度准确的癌症标志物,其恶性风险>99%,更重要的是在BRAF突变阳性的样品中,有40%的样品有不确定的FNA细胞,这表明BRAF在评估这些结节时的诊断价值较高[31-32]。


2. RAS基因突变:HRAS、KRAS和NRAS基因编码位于细胞膜内表面的高度相关的g蛋白,经MAPK、PI3K、AKT等信号通路传递细胞膜受体酪氨酸激酶和g蛋白偶联受体产生的信号。在甲状腺癌中,NRAS 61号密码子和HRAS 61号密码子突变最为常见。在10%~20%的PTC、40%~50%的FTC及20%~40%的PDTC和ATC中均发现RAS基因的突变,在带乳头状细胞核特征的非侵袭性滤泡型甲状腺肿瘤(non-invasive follicular thyroid neoplasm with papillary like nuclear features,NIFTP)的突变率为20%~40%,59%伴有RAS突变的病例为NIFTP[33-39],但在20%~40%的良性滤泡性腺瘤中发现RAS,提示ras阳性滤泡腺瘤可能是ras阳性FTC和滤泡型乳头状癌(follicular variant papillary thyroid carcinoma,FVPTC)的前体,并且RAS突变可能使分化良好的甲状腺癌更易发生去分化和间变性转化[40-43]。研究结果表明,RAS合并其他基因改变,特别是TERT启动子、EIF1AX或TP53突变时,预示高侵袭甲状腺癌的风险明显增高,在考虑治疗策略和手术范围时宜更为积极[44]。


3. RET或PTC重排:10%-20%的PTC可发生RET或PTC重排,并具有这种肿瘤的特异性[45-46],高达80%的暴露于意外辐射(主要是放射性碘)或治疗性辐射(主要是体外放射治疗)的PTC存在RET或PTC重排[47-49]。常见的有RET或PTC1和RET或PTC2两种形式的重排[50-51]。回顾性研究结果显示,RET或PTC重排对Bethesda Ⅲ类结节恶性诊断率为60%,提示RET或PTC重排是PTC诊断特异性较高的标志物。此外,50%的MTC伴有RET基因突变,RET基因胚系突变造成MAPK和PI3K通路持续激活,尤其是密码子918位点突变,约98%的多发性内分泌腺瘤病2A型(multiple endocrine neoplasia type 2A,MEN2A)患者存在RET基因胚系突变[52]。约88%的家族性MTC含有RET基因的突变,约6%的散发性MTC含有胚系的RET基因突变,NCCN指南中指出对于遗传性MTC需要进RET基因的检测[10]。


4. TERT基因突变:TERT突变在DTC中的发生率为10%~15%,在PDTC和ATC中为40%~45%,而在良性结节中比较罕见[53]。Liu等在一项大型中国人群PTC和FTC队列研究中发现,TERT启动子突变常见,特别是在FTC和BRAF突变阳性的PTC中,并且与临床病理特征相关。与BRAF突变不同,TERT启动子突变与高碘摄入无关,但与患者年龄较大、肿瘤体积较大、甲状腺外侵及晚期PTC相关。同时存在TERT和BRAF突变与临床病理侵袭性更常见,提示TERT突变BRAF突变共存时,PTC侵袭性及复发风险显著增加,因此TERT基因是甲状腺癌诊断和侵袭性评估重要的肿瘤标志[54-55]。


5. TP53基因突变:研究发现TP53的突变主要存在于71%的ATC及25%低分化的PTC中,在有分化程度不同的2个区域(分化程度高的区域和分化程度低)的ATC中发现TP53突变仅发生在分化程度低的区域,这表明TP53基因的突变与低分化组织学类型的甲状腺肿瘤,如ATC和PDTC高度相关,因此认为其在PTC由分化型向未分化型转化的过程中起关键作用[29, 56],这提示TP53的失活可能赋予这些肿瘤的侵袭性,并进一步失去分化功能[57]。


6. PAX8/PPARγ重排:PAX8/PPARγ重排在FTC中是一种很常见的突变,发生率为30%~35%[58-60]。研究结果表明,约2%~13%的滤泡腺瘤和1%~ 5%的FVPTC中也发现了这种重排[61-62]。PAX8/PPARγ重排和RAS点突变在同一肿瘤中很少并行发生,提示其在FTC发展过程中代表不同的发病途径[59, 63]。


7. mRNA和miRNA表达:在乳头状癌和其他类型的甲状腺癌中观察到不同的mRNA表达谱[64]。尽管在不同的研究中特定的基因表达谱的列表有很大差异,但已重复发现了一些共通的基因在mRNA水平上的失调,包括MET、TPO、TIMP1、DPP4、LGALS3、KRT19、HMGA2、PLAG1[65-72]。mRNA和miRNA(microRNA)标志物的表达也可用于甲状腺FNA样本的诊断评估。研究结果表明,利用NRAS突变状态结合TIMP1表达分析,一项评估综合临床因素和候选诊断标志物以提高甲状腺细针穿刺活检准确性的前瞻性研究测试了在甲状腺FNA样本中联合检测常见体细胞突变(BRAF V600E、NRAS、KRAS、RET/PTC1、RET/PTC3、NTRK1)和6个候选基因(ANGPT2、ECM1、EGFR、EFNB2、TIMP1、TMPRSS4)的表达水平,通过对341个甲状腺FNA样本应用商业开发的算法,区分甲状腺良恶性肿瘤的总体准确率为91%,FNA不确定组67%,FNA可疑组77%,可进一步提高FNA细胞学诊断的准确性[73]。Thomas等[74]一项大型前瞻性数据的研究结果表明,miR-21表达的增加与高侵袭性PTC(高细胞变异)相关,可能是其发病机制中的一个关键事件。Labourier等[75]的大型数据研究结果提示,平台检测DNA、mRNA、miRNA可准确划分甲状腺结节的良恶性,提高分子细胞学的诊断率,进一步提高AUS/FLU或FN/SFN细胞学对良性结节的术前风险管理。


8. 其他分子标志物:其他的分子标志物,如PI3K信号通路的成员,如PTEN、PIK3CA和AKT1也有少量报道[23]。在未分化和低分化癌中存在10%~20%PIK3CA突变、5%~15%的PTEN突变和5%~10%的AKT1突变[76-80]。另一个在ATC中经常突变的基因是CTNNB1,60%的ATC中可发现该基因3号外显子点突变,这些突变也发生在低分化甲状腺癌中,但发生率较低[81-82]。一项包含多个热点突变检测的高通量测序研究发现,在ATC中CDKN2A也是常见的突变类型,其中最常见的是E88G错义突变,在8/118(7%)的ATC样本中检测到[83]。研究发现除了驱动突变,还有其他基因(CHEK2、ATM)和一系列功能相关基因的突变,可能导致甲状腺结节分化缺失和肿瘤进展[74]。此外,还有研究结果表明EIF1AX突变是与RAS在晚期甲状腺癌中的强合作事件。在一项对246例甲状腺肿瘤患者的研究发现有26例(11%)含有EIF1AX突变,其中25例与RAS突变相关[84]。

除了上文提及的分子标志物,约30%~40%的分化型甲状腺癌没有检测到任何这些已知基因突变,使得这些标志物的敏感度低得不可接受[23]。在一些研究中,某些基因突变也会发生在良性甲状腺结节中,发生率为6%~13%,可能会影响这些基因突变检测的诊断特异性[85-87]。


二、甲状腺癌基因检测应用进展


在过去的十年中,分子诊断检测已用于指导FNA不确定的甲状腺结节患者的管理,并最终减少以诊断为目的的甲状腺手术的数量[53]。得益于高通量测序的发展,越来越多新的甲状腺癌分子标志物的发现,极大地提高了对甲状腺癌分子发病机制的认识,从而为甲状腺癌患者提供了更加个性化的治疗。


临床应用的主要目的是评估甲状腺结节的良恶性,分子检测panel主要分为排除疾病在外“rule out”和排除疾病在内“rule in”,“rule out”评估指标为敏感性,即阴性预测值(negative predictive value,NPV);“rule in”评估指标为特异性,即阳性预测中(positive predictive value,PPV)的能力。敏感性和特异性评估检测特征独立于人群中的疾病发生率,而NPV和PPV依赖于人群中的疾病发生率。不同研究对不同人群的敏感性和特异性可以直接比较,而NPV和PPV不能直接比较。要充分评估NPV和PPV,需要了解每个Bethesda细胞学分类的恶性肿瘤的患病率,最好是在3~5年的时间内[88]。甲状腺结节患者分子标志物临床验证试验最可靠的设计是对患者进行连续性的前瞻性、双盲性试验,并对所有接受手术的患者进行组织病理学确认,以及对分子诊断为良性结节患者的长期随访研究[89-90]。现将目前常见的、甲状腺癌基因检测产品的优势和局限性总结如下(表2)。

1. ThyGenX:“rule in”:基于高特异性和高PPV,Asuragen公司的ThyGenX 7基因Panel已被证明是一种有效的“rule in”甲状腺恶性肿瘤细胞分子检测,使用多重聚合酶链式反应方法对7个基因(BRAF V600 E、NRAS 61号密码子、HRAS 61号密码子、KRAS12/13号密码子点突变和RET/PTC1、RET/PTC3、PAX8/PPAR重排)进行特异性探针测序检测,每次检测的价格为1675美元[91]。针对该panel的最大研究是一项单中心、非盲性前瞻性研究,包含了1056个FNA不确定结节和513个有手术病理的结节(Bethesda Ⅲ类247例,Bethesda Ⅳ类214例,Bethesda V类52例),总体特异度为98%。对于AUS/ FLUS、FN/SFN和SMC的每一种不确定类型的特异性分别为99%、97%和96%。但该方法的总体敏感性仅为60%,在AUS/FLUS、FN/SFN和SMC中分别有37%、43%和32%的癌症无法诊断。该研究结果表明,基于细胞学诊断的所有类型的FNA细胞学诊断不确定样本,包括AUS/FLUS、FN/SFN和SMC的恶性风险分别为14%、27%和54%。如果在标本中发现任何突变,AUS/FLUS、FN/SFN和SMC的恶性风险分别增加到88%、87%和95%[92]。尽管Asuragen公司已将该检测panel商业化,但尚未发表该公司对该方法的验证结果。某些医学研究中心建议将这些标志物作为FNA标本中甲状腺癌的前瞻性预测因子进行非商业性检测使用[86, 92-93]。由于ThyGenX的灵敏度低,因此不是一个有效的“rule out”检测。此外,ThyGenX 7基因panel阳性的临床意义是引导患者接受甲状腺切除术而不是肺叶切除术,在2015年ATA制定的“甲状腺结节和分化型甲状腺癌患者指南”[5]中指出,2步外科手术程序不再是常规要求之后,在目前的临床实践中,ThyGenX 7基因突变panel在外科手术决策中的作用减弱,主要是因为该检测的敏感度较低[53]。


2. Afirma GEC:“rule out”:2011年,Veracyte公司的167基因表达分类器Afirma GEC基于mRNA表达和专有算法,根据mRNA表达模式来区分良性结节和恶性结节,用于不确定细胞学的甲状腺结节的诊断评估,已用于临床,每次检测的价格为4875美元(Afifirma GEC&MTC)[91, 94]。一项用GEC对包括4812份FNA样本,其中577份(12%)FNA不确定,265份FNA疑似样本进行的前瞻性、双盲、多中心临床验证分析,对所有FNA不确定的甲状腺结节样本(Bethesda Ⅲ/Ⅳ/Ⅴ)的敏感度和特异度分别为92%和52%,AUS/FLUS的敏感度和特异度分别为90%和53%,FN/SFN样本的敏感度和特异度为90%和49%。鉴于AUS/FLUS结节的恶性肿瘤患病率为24%,FN/SFN结节的恶性肿瘤患病率为25%,这项研究对AUS/FLUS和FN/SFN的NPV和PPV分别为95%和38%,94%和37%[89]。另一项使用Afirma GEC的研究对标准方法采集到的FNA样本进行分析前、分析性能和重现性研究。该方法宣称能够在10ng起始RNA投入量的情况下准确地区分良性和可疑样本。该检测的准确性能在20%FNA与正常组织稀释混合样本,以及17%FNA与全血稀释混合样本中重复验证。该方法的批内精密度为94%,重复性为100%(实验室间)[95]。关于GEC的验证研究还有很多,总之,当恶性肿瘤的人群患病率在小于或等于验证队列时,GEC的高敏感性支持将其作为Bethesda Ⅲ/Ⅳ类结节甲状腺癌的“rule out”检测。然而GEC的低特异性和PPV意味着阳性检测不足以预测结节是否恶性的,后续的临床决策必须结合细胞学结果、超声检查结果和临床危险因素综合考虑,此外因为Bethesda V类结节的恶性肿瘤患病率相对较高,在这种情况下GEC的临床有效性较低,不推荐使用GEC[96]。

基因组测序分类器Afirma GSC是Afirma GEC的替代升级版,对593个基因的RNA测序,已经过分析验证(敏感度为91%,特异度为68%),但尚未发表(Kennedy GC, personal counication)[53]。


3. ThyraMIR:“rule in”和“rule out”:ThyrGenX/ThyraMIR联合推出了结合了ThyGenX 基因突变panel和小的非编码microRNAs(miRNAs)表达的分类器,包含了5个基因突变和3个融合基因转录的靶向测序以及10个miRNA的表达,当ThyGenX基因突变检测为阴性时,推荐进一步进行ThyraMIR检测,价格为3300美元[91]。临床验证研究包括109个Bethesda Ⅲ/Ⅳ类甲状腺结节(32%的恶性肿瘤患病率)队列,敏感度为89%,特异度为85%,NPV为94%,PPV为74%[75]。ThyGenX/ThyraMIR这种综合测试提高了检测的总体精密度,被推广为一种高PPV和高NPV的综合测试,目前还没有太多的ThyGenX/ThyraMIR验证后的研究报道。

ThyroSeq:“rule in”和“rule out”:同样是使用下一代测序方法,CBLPath的ThyroSeq在ThyroSeq V0 7基因Panel上进行了扩展,ThyroSeq作为一种综合检测,兼顾了“rule in”和“rule out”的检测能力。ThyroSeq V1是一个包含12个与甲状腺癌相关的基因的284个突变热点的定制panel。一项针对ThyroSeq V1的研究测序分析了228个甲状腺肿瘤和非肿瘤样本的DNA,包括105个冷冻样本、72个福尔马林固定样本和51甲状腺癌细针穿刺样本,只需5~10ng输入DNA就足以成功分析99.6%的样品,突变检测的分析准确率为100%,突变等位基因的敏感度为3%~5%,其中145例甲状腺癌中的110例(76%)和83例良性标本中的5例(6%)发现了突变[97]。随后的ThyroSeq v2(14个基因点突变和42个基因融合),在V1的基础上增加了端粒酶逆转录酶(TERT)基因启动子突变,检测38种RET融合基因,并对8种基因进行表达分析,以充分区别非甲状腺病变(如甲状旁腺)。一项最大的独立评估ThyroSeq v2的验证后研究,包括了190个Bethesda Ⅲ/Ⅳ类甲状腺结节,其中102个结节经手术组织学检查,其总体敏感度为70%,特异度为77%,NPV为91%,PPV为42%,其中Bethesda Ⅳ类结节的检测性能明显优于Ⅲ型结节[98]。最新版包含112个基因panel的ThyroSeq V3,评估基因点突变、基因融合、拷贝数变异(CNV)以及基因表达,能够在细胞学不确定的FNA样本中对多个基因进行检测诊断,价格为3200美元[91]。在用ThyroSeq V3进行的一项前瞻性、双盲、多中心队列研究中,包括257个FNA不确定甲状腺结节,其中61%的结节检测结果为阴性,在这些结节中只有3%的癌残留风险,即高达61%的FNA不确定的甲状腺结节患者可以通过ThyroSeq V3检测避免诊断性手术,RAS突变是最常见的阳性结果[90, 99]。另一个ThyroSeq v3对238例手术组织标本(包含205例甲状腺癌样本)和175例Bethesda Ⅲ/Ⅳ类样本(52.6%的恶性肿瘤患病率)进行分析验证,准确地检测了低于2.5ng起始DNA投入的基因突变,准确地分类了在88%的正常甲状腺细胞中有12%肿瘤的10个样本,并在3个稀释到12%的血液样本中准确重复检出。研究结果显示训练集的敏感度、特异度和准确度分别为94%、89.4%和92.1%,验证集分别为98%、81.8%和90.9%[100]。一项最新的对286例不确定Bethesda Ⅲ-V类甲状腺结节的前瞻性、双盲、临床验证研究结果显示,Bethesda Ⅲ类结节的敏感度和特异度分别为91.4%和84.9%,Bethesda Ⅳ类结节的敏感度和特异度为97%和75%。Bethesda Ⅲ类(23%的恶性肿瘤患病率)的NPV和PPV分别为97.1%和 64%,Bethesda Ⅳ类结节(35%的恶性肿瘤患病率)的 NPV 和 PPV 分别为98% 和68%[90]。总之,ThyroSeq采用下一代测序的方法只需少量的DNA投入量即可同时检测多个突变,具有较高的准确性和敏感度。


5. mir-THYpe:“rule in”和“rule out”:mir-THYpe测试可以认为是一种综合性恶性肿瘤的“rule in”和“rule out”检测,分析检测11个miRNAs的表达谱。一项mir-THYpe的研究分别分析了39个良性和39个恶性甲状腺样本(FNA不确定)中的96个miRNA候选基因的表达,作为训练mir-THYpe的训练集,58个良性、37个恶性FNA涂片(FNA不确定)作为验证集,在训练集中仅使用11个miRNAs进行10次交叉验证,mir-THYpe检测的敏感度为89.7%,特异度为92.3%,NPV为90.0%,PPV为92.1%。在FNA涂片验证组中,mir-THYpe检测的敏感度为94.6%,特异度为81.0%,NPV为95.9%,PPV为76.1%[101]。mir-THYpe可以使用现有的细胞学切片,成本显著降低。目前尚缺少开展评估mir-THYpe在临床表现的前瞻性研究及在临床实践中的常规使用及其成本效益数据。


6. ThyroidPrint:“rule in”和“rule out”:GeneproDx公司的ThyroidPrint是一种基于多重定量聚合酶链反应(qPCR)法的体外诊断(in vitro diagnostic,IVD)基因分类器,可以准确地分类甲状腺结节。在有关ThyroidPrint的一项针对FNA样本的前瞻性多中心队列的研究,在组织活检样本中使用10个基因的最佳分类器在9个交叉验证测试集中达到了最佳和一致的性能(敏感度为93%,特异度81%)。在FNA样本的多中心队列中,构建在一个多重qPCR的IVD分类器中的10个基因,PPV为78%,NPV为98%。在恶性肿瘤患病率为20%~40%的患者中,PPV为64~82%,NPV为97 ~99%[102]。


7. 其他应用:TSHR mRNA是由克利夫兰诊所实验室(Cleveland Clinic Laboratories)开发的,使用基于定量逆转录PCR的方法检测外周血中TSHR mRNA水平。甲状腺激素受体(TSHR)mRNA检测虽然不是甲状腺细胞学诊断的标志物,但也有助于对细胞学上不确定的甲状腺结节进行分类[103]。一项研究结果表明,TSHR mRNA的敏感度为75%,特异度为78%。TSHR mRNA和FNA细胞学联合诊断的性能特征为:敏感度为95%,特异度为83%,PPV为84%,NPV为95%,所有的数值均明显高于单独从FNA细胞学诊断得到的结果[104]。一项研究使用mRNA表达分析检测了315个甲状腺结节中超过247,186个转录本,包括多个亚型。数据集包括178例回顾性手术组织和137例前瞻性采集的FNA样本。两个分类器分别在手术组织和FNA上训练,应用商业开发的算法,研究人员能够以92%的敏感度和84%的特异度区分良性和恶性结节(NPV估计为96%),且即使在良性背景稀释80%的情况下,分类器也能正确识别大多数FNA样本中的恶性结节[105]。


另一种miRNA分类器检测是Rosetta GX Reveal,是一种基于miRNA表达来诊断FNA不确定甲状腺结节的检测方法,检测24个miRNA的表达,其独特之处在于可在现有的FNA细胞学涂片上进行检测[106]。一项对新型Rosetta GX Reveal检测方法的双盲、回顾性、多中心验证研究,评估了其对配有配对手术样本的189份FNA样本的性能(Bethesda Ⅲ类结节),总体性能为:敏感度为85%,特异度为72%,PPV为59%,NPV为91%[107]。


三、总结和展望


甲状腺分子检测的最终目的是在术前准确评估甲状腺结节的性质,降低细胞学不确定甲状腺结节的诊断不确定性,不同的检测方法显示出不同的特点和优势。这些研究与应用强调了基因检测在甲状腺癌管理治疗方面的巨大潜力,且在下一代测序和mRNA表达方面的应用居多,下一代测序的优势是可以同时并行检测多个分子标志物且具有更高的基因突变分辨率,综合性比较强。基因表达分析,包括mRNA和miRNA,可用于大组合,提供肿瘤的基因组指纹,可用于术前预测哪些结节最终是良性或恶性,这种方法非常强大,但取决于所使用的方法和训练集的广度。同时,有研究结果表明甲状腺结节每年的恶性率之间存在着显著的差异[88]。由于分子诊断检测的NPV和PPV依赖于被评估人群中的恶性肿瘤的患病率,因此持续的联合病理学评估以指导分子检测结果的解释和临床决策至关重要[53],目前伴随使用分子检测来指导治疗决策的长期结果数据仍然不足。最后值得一提的是,在这些研究中,国内的相关前瞻性以及临床验证性研究相对较少,尤其缺少大型中国人群数据的研究。


总之,甲状腺结节分型分子标志物、肿瘤预后及分子治疗靶点的选择,在FNA细胞学检查不确定的结节癌的诊断方面已经取得了实质性的进展。我们相信,甲状腺癌分子生物学的进一步发展将消除这些不确定的诊断类别,这将对甲状腺结节患者的临床管理产生深远的影响,未来的发展将是一个前沿的、需要多检测平台交叉融合,极具挑战性且生机勃勃的领域。
