自配试剂免疫透射比浊比率法测定血清CIC_检验世界网

  【摘要】  目的:寻求一种全自动仪测循环免疫复合物(CIC)的新方法,以取代传统方法繁琐的手工劳动和略显粗糙的定量结果。 方法:自配PEG和BBS试剂,以康宁560全自动生化仪结合AU560全自动生化仪设置程序最佳参数,免疫透射比浊终点法多点定标PEG和BBS,比率法计算CIC值,以百分回收试验和重复性试验确定其准确度和精密度,以对照试验与传统PEG沉淀法作对比分析。结果:仪测CIC最佳参数设置为:血清:试剂=1∶8,λ1340 nm,λ2450 nm,内源性读数时间20 Sec,总体反应时间288 Sec,总反应体积为180 μl,37℃。本方法与传统PEG沉淀法的回收率、日内和日间变异系数(CV%)分别为97%~110%、3.5%、4.3%和90%~122%、6.8%、9.7%。与传统PEG沉淀法相比,准确性差异无显着性(P>0.05),两者具有较好的相关性(r=0.852),精密度则比手工方法高。 结论:自配试剂上机检测CIC,其操作简便易行,且定量相对准确,重现性好,线性范围宽,完全可以取代繁琐的传统操作。

  Serum CIC Determination By Immunological Transmission Turbidimetry Ratio with Selfmix Reagents

  Abstract: Objective To look for a new  fullautomatic instrumental method for Circulating Immune Complexes (CIC) determination,to replace the tedious handicraft labor of traditional method and  gain more accurate  results.Methods Try to mix  BBS and  PEG reagents self,establish the best procedure parameters with Kangning 560 combine with   AU560,immunological transmission turbidimetry  4 points calibration  the PEG and  BBS,compute the CIC value with ratio method,then judge its accuracy and degree of precision by percentage recovery ratio study and repeatability study,make contrast experiment to compare with traditional PEG precipitation method. Results The  best procedure parameters of CIC instrumental method were:Serum:Reagents=1:8,λ1340 nm,λ 2450 nm,endogenous read time 20 Sec,total reaction time 288 Sec,total reaction volume 180 ul,37 ℃. Percentage recovery ratio,inside day and daytime percentage variation coefficient(CV%) of this method and traditional PEG precipitation method respectively for 97% ~110%,3.5%,4.3% and 90 %~122%,6.8%,9.7%. Compared with traditional PEG precipitation method,it has no difference in accuracy(P>0.05),the two have good correlation(r=0.852),but degree of precision is higher than handicraft method.Conclusion Serum CIC determination by immunological transmission turbidimetry  ratio  with  Selfmix reagent,its operation is simple and convenient,also it shows quantitative measurement relatively accurate,good repeatability,wide linear,it may replace nittygritty traditional handoperation completely.

  检测血清循环免疫复合物(CIC)传统的方法是PEG沉淀法,该方法需4℃冰箱1 h,室温45 min,而后以BBS管调零 λ500 nm测PEG管,查标准曲线得出结果[1]。整个操作过程均为繁琐的手工劳动,其定量结果也略显粗糙。将CIC检测搬上全自动仪器,是一个亟待解决的课题。近年来,随着大型全自动生化仪的逐渐普及,以仪测代替手工劳动已成为检验界一个大发展趋势。免疫透射比浊法更以其简捷、灵敏、准确、重现性好和易于在全自动化仪器上进行大批量检测的优点更多地为人们所采用。我们利用自配试剂在康宁560全自动生化仪上建立了CIC测定,使CIC繁琐的传统检测变得简单易行,保证了准确性,提高了精密度,获得较满意的结果。

  1  材料和方法

  1.1  试剂

  BBS试剂:硼砂(Na2B4O7·10H2O)AR  8.59g,硼酸AR 6.82g,加水至1 000 ml;PEG试剂:用聚乙二醇(AR)4.167 g加BBS试剂至1 000 ml,NaN3防腐。

  1.2  CIC定值血清

  鉴于目前国内市场未见有CIC定值血清出售,自制混合血清,以传统PEG沉淀法重复测定20次,统计结果,以(±s)表示,其中X作为定值血清靶值。本实验中,定值血清为(291.0±14.0)U,以生理盐水配制成4点定标系列(0,72.8,145.5,291.0 )。

  1.3  仪器

  康宁560(美国)为测试仪器;Au560(日本)辅助设置参数。
